We are seeking a dedicated and empathetic Community Social Worker to join our team. Our approach focuses on connecting individuals to their community and social networks, empowering them to problem-solve collaboratively and develop person-centred plans that promote independence, choice, and control. We adhere to legislative frameworks and national/local policies while prioritising the safety and well-being of those we support. You will play a crucial role in supporting individuals, including advocates and carers, to connect with their community, develop person-centred plans, and navigate legal and affordable support options. You will uphold the principles of the Mental Capacity Act (2005) when working with individuals lacking mental capacity, ensuring their safety and well-being while respecting their autonomy. Collaboration, documentation, and adherence to safeguarding procedures are integral to this role.
Responsibilities Include:
1. Apply a strength-based approach to connect individuals to their community/social network and problem-solve together.
2. Work with individuals, advocates, and carers to develop person-centred plans that maximise independence, choice, and control, ensuring plans are legal and affordable.
3. Act in the best interest of individuals lacking mental capacity, using the principles defined in the Mental Capacity Act (2005) to maintain their safety and well-being.
4. Provide support for the appropriate arrangement and implementation of person-centred plans, ensuring individuals receive the necessary support and services.
5. Work within the legislative framework, national and local policy, and the philosophy of social work practice, documenting all work and sharing information in line with legislation and local policy and procedures.
6. Participate in the Duty Safeguarding Manager rota, conducting timely and effective safeguarding assessments and planning.
7. Carry a caseload of a volume and complexity as designated by the supervisor, preparing reports for legal proceedings when appropriate.
8. Take an active role in your own supervision and appraisal, and supervise and appraise allocated staff in accordance with departmental policies.
9. Be professionally accountable for all aspects of your work, providing appropriate support and challenge to designated staff, enabling continuous professional development and evidence-based practice.
The successful candidate will be:
10. A Qualified and Registered Social Worker
11. Experienced with at least 5 Years of experience with older people
