Nursing Associate - ( Liberty PCN Aligned Community Team PACT)

Are you passionate about improving care for older adults living withfrailty? We are looking for a dedicated Nursing Associate (Frailty) tojoin our PCN Aligned Community Team (PACT), based at the St GeorgesHealth & Wellbeing Hub in Hornchurch. You'll play a keypart in proactive frailty support, undertaking holistic assessments, supportinganticipatory and personalised care plans, and working closely with a broadmultidisciplinary team to ensure seamless, patient centred care.

The post holder will work with other Healthcare professionals and key partners with the PCN aligned community team (PACT) in Havering Liberty PCN.

In line with the Havering Place based Partnerships priorities of Age Well and Die Well, the PACT team aims to support older individuals living with frailty or complex needs to live longer and healthier lives. PACT aims to achieve this by:

* providing a single point of contact for individuals and their carers
* supporting the proactive undertaking of comprehensive geriatric assessment to identify holistic needs and develop high quality, personalised, anticipatory care
* coordinating the provision of holistic, integrated care, through collaboration with other services/professionals, e.g. specialist community teams, social prescribers, voluntary sector organisations, GPs
* supporting the transition from hospital to home through consultation and collaboration with acute care partners


About us

Havering Liberty Primary Care Network brings together five GP practices across Upminster and Hornchurch, working collaboratively to serve a diverse population with highquality, accessible and patientcentred primary care. Supported by an extended multidisciplinary team, we focus on improving the health and wellbeing of both our community and our workforce through proactive, personalised and integrated care.

Our PCN is led by our Clinical Director and Leadership Team, working closely with GP leads and practice colleagues to deliver coordinated services and drive meaningful improvements across the network. We are supported by our GP Federation, Havering Health Ltd, who act as the employing body for our workforce and help ensure the delivery of innovative, highquality and responsible healthcare services for the residents of Havering.


Job responsibilities

Key Responsibilities:

The post holder will be an experienced Registered Nursing Associate working alongside the clinical and non-clinical PACT members, collaborating with colleagues from primary care, community and secondary care, as appropriate. They will assist coordination of all key activity including access to services, advice, information, and ensuring health and care planning is timely, efficient, and patient-centred The post holder will be based at a primary care site and will routinely visit patients in their own homes; they may at times be expected to work in other settings, e.g. clinic setting.

Clinical Practice:

The Nursing Associate will visit patients in their homes to:

Undertake and record clinical observations including blood pressure, pulse, temperature, oxygen saturation, height, weight, BMI.

Perform venipuncture, capillary blood sampling and other diagnostic procedures

Carry out ECGs.

Support chronic disease monitoring (e.g. asthma, COPD, diabetes, hypertension) under supervision.

Support patients with long-term conditions, including those with complex needs such as dementia and mental health conditions.

Undertake NHS Health Checks.

Administer vaccinations and immunisations under Patient Group Directions (PGDs) or Patient Specific Directions (PSDs), (if appropriately trained and assessed as competent).

Recognise and respond appropriately to deteriorating patients or safeguarding concerns.

Provide evidence-based lifestyle advice (smoking cessation, weight management, alcohol awareness, physical activity).

Promote self-management of long-term conditions.

Signpost patients to appropriate services.

Support national and local public health initiatives.

Clinical Governance & Quality:

Maintain accurate, contemporaneous patient records in line with practice policy.

Support Quality and Outcomes Framework (QOF) achievement.

Participate in audit, data collection and quality improvement activities.

Contribute to infection prevention and control procedures.

Adhere to CQC standards and practice policies.

Participate in significant event analysis and learning reviews.

Maintain cold chain compliance for vaccines.

Professional Responsibilities:

* Maintain active registration with the Nursing and Midwifery Council (NMC).

Work within the NMC Code and scope of professional practice.

* Maintain revalidation requirements.

Education and Development:

It is the responsibility of the employee to comply with all organisational and statutory mandatory requirements (e.g. health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance) and completing the required training.

Engages in annual appraisal, developing objectives to inform a Personal Development Plan, which may include 360‑degree appraisal and use of patient feedback.

Maintain own personal and professional development through education, training and networking.

Undertake education and training that is appropriate to the role of Nursing Associate, as identified in Professional Development Plan.

Support and mentor Healthcare Assistants, Student Nursing Associates and Nursing Apprentices, and those on learning assignments/placements as required

Communication:

Communicate with other health care professionals and address interface issues within primary, community and secondary care to ensure consistent and accurate transfer of information relating to patient care.

Work collaboratively with the PCN MDT to ensure delivery of nursing associate duties complement existing workforce.

Communicate effectively regarding patient care and elevate concerns appropriately.

Attend and contribute to practice and PCN team meetings where appropriate.

Work closely, and demonstrate empathy with clients, carers and their families to agree decision making relevant to care management particularly where barriers to understanding exist.

Employ excellent written and verbal communication skills.

Demonstrate skills in motivating clients and/or carers to engage in the care process.

Form productive relationships with others who may be under stress and/or have difficulty with their communication.

Raise any concerns to a registered care professional or appropriate person

Working Conditions

This post requires the post holder to travel to the PCN registered housebound patients, as well as travel from St Georges Health and Wellbeing Hub, to PCN practices and to other venues during fulfilment of their duties.
