Job Title: Support Worker – Supported Living (Learning Disabilities) -Location: Community-based / Supported Living Services -Hours:Flexible (Days, Evenings, Weekends, Sleep-ins where applicable) -Contract Type: Full-time / Part-time / Bank -Reporting to: Service Manager / Team Leader Job Purpose To provide person-centred support to adults with learning disabilities living in supported living accommodation, enabling them to live as independently as possible within the community. The role focuses on promoting choice, dignity, inclusion, and wellbeing, while supporting individuals to achieve their personal goals and live fulfilling lives. Key Responsibilities Person-Centred Support * Deliver high-quality, person-centred support in line with individual support plans, risk assessments, and outcome goals. * Promote independence, choice, and control in all aspects of daily life. * Support individuals to develop and maintain life skills, including cooking, budgeting, cleaning, and personal organisation. Community Inclusion * Support individuals to access community activities, education, volunteering, employment, and social opportunities. * Encourage and facilitate positive relationships with family, friends, and the wider community. * Support individuals to use public transport and community facilities safely and confidently. Personal Care & Daily Living (where applicable) * Provide support with personal care in a respectful and dignified manner, where required. * Support with meal planning, shopping, food preparation, and healthy eating. * Support with medication in line with training, policies, and individual risk assessments. Health, Wellbeing & Safety * Monitor physical and emotional wellbeing and report any changes appropriately. * Support individuals to attend health appointments and follow agreed healthcare plans. * Implement positive behaviour support strategies where required. * Follow safeguarding procedures and report any concerns immediately. Communication & Record Keeping * Maintain accurate and timely records, including daily notes, incident reports, and outcome tracking. * Communicate effectively with colleagues, managers, families, and external professionals. * Contribute to reviews of support plans and risk assessments. Professional Practice * Work in line with CQC Fundamental Standards, organisational policies, and relevant legislation. * Maintain confidentiality at all times. * Participate in training, supervision, and appraisal. * Work flexibly, including lone working, evenings, weekends, and bank holidays as required. Essential Criteria * A genuine commitment to supporting adults with learning disabilities. * Values aligned with promoting independence, dignity, and inclusion. * Good communication skills (verbal and written). * Ability to work independently and as part of a team. * Willingness to undertake training and development. * Right to work in the UK. * Enhanced DBS check (or willingness to obtain one). Desirable Criteria * Previous experience in supported living, learning disabilities, or social care. * NVQ/QCF Level 2 or 3 in Health & Social Care (or willingness to work towards). * Experience supporting individuals with autism, complex needs, or behaviours that challenge. * Full UK driving licence (where required by the service).
