Are you passionate about mental health care and looking for an exciting opportunity to join a supportive community mental health team? We are currently seeking a Social Worker or Occupational Therapist to become part of our dedicated Community Mental Health Team based at the Whiteleaf Centre in Aylesbury. Our team is committed to providing exceptional care and treatment to adults with mental health challenges and supports staff in their professional growth.
Responsibilities
* Work as an autonomous practitioner within a multidisciplinary team to perform comprehensive assessments in collaboration with service users and their families/carers.
* Partner with service users and their families/carers to create and execute treatment plans that ensure safety, promote well‑being, and support independence and community inclusion.
* Provide interventions following NICE guidelines to promote recovery and prevent relapse in various settings, regularly reviewing care and outcomes.
* Support and supervise junior team members, act as a mentor/preceptor to student nurses/trainees, and participate in clinical education and practice development initiatives.
* Engage in operational planning, policy implementation, and service development within the team, including leading on delegated projects and participating in research and audit projects.
Qualifications
* Full valid driving licence and access to a vehicle for work.
* For international candidates: SW England or HCPC registration required.
