Overview
The Adult Social Care Transitions Team (14-25) is a specialist service that supports young people as they move from childhood into adulthood. This team offers a unique opportunity to work across learning disability, neurodiversity, physical disability, and mental health contexts. You'll develop relationship-based practice and work closely with Children\'s Services to ensure a seamless transition and a tailored support package when a young person turns 18. The team is made up of Social Workers and Social Care Practitioners, all focused on supporting young people, their families, and the people who are important to them. We also work collaboratively with professionals such as Children\'s Social Workers, SEND workers, and education and health professionals. Our team maintains strong links with schools and engages with SEND and education provisions to support effective transitions. Our work is both complex and rewarding, often involving Court of Protection referrals and other high-level interventions. As a Senior Social Worker, you will play a key role in supporting Social Workers and Social Care Assessors to deliver best practice. You\'ll help ensure that interventions are smooth, positive, and least restrictive, in line with our commitment to person-centred care. This role also offers the opportunity to be involved in safeguarding and project work that directly contributes to Staffordshire County Council\'s vision for young people, helping to shape services that are responsive, inclusive, and empowering. We welcome applications from Approved Mental Health Professionals (AMHPs), Best Interests Assessors (BIAs), or individuals who are willing to work towards these qualifications. We\'re happy to discuss flexible working to support a healthy work-life balance. This role qualifies as an essential car user, reflecting the need for mobility across the county.
