JOB PURPOSE
To provide and supervise the delivery of high quality nursing care to residents and maintain professional and clinical standards.
DUTIES AND RESPONSIBILITIES
QUALITY AND REGULATION
1. To assess the resident's physical, psychological and social needs and compile a care plan in collaboration with the resident, family and any other health care professional which is outcomes based.
2. Plan. Implement and evaluate the nursing care in relation to the resident's dynamic needs in accordance with clinical policy and regulatory framework with other appropriate health care professionals.
3. Ensure all entries in the Nursing Documentation comply with the NMC, The Code, GDPR 2018, CIW Standards (Statement of Purpose) Aneurin Bevan University Health Board / Social Services Commissioning Contracts. All entries are to be dated, time and full signature ensuring all entries are completed in a timely manner throughout the day / night.
4. Participate in the formal audit programme within your level of competence.
5. Maintain Registration with the NMC and have an active Pin Number
6. Comply with Revalidation programme and all areas of the Code which governs the Nurse's practice.
7. Be familiar with Resident of the Day programme.
CLINICAL SKILLS & KNOWLEDGE
1. The resident should be visited regularly during the shift by the registered nurse to ensure appropriate nursing care is being delivered in a professional and timely manner by the care team under your supervision.
2. Comply with NICE Guidelines on the Administration of Medication and clinical policy to ensure that the processes are followed to ensure the safe ordering, custody, storing, disposal, recording and administration of all medication.
3. Management of stock levels and replenishment of all appropriate clinical supplies. Ensure there is sufficient equipment specific to the individual resident's needs. To have an understanding surrounding Health and Safety and risk assessment within the service e.g. handling and disposal of sharps, contaminated equipment, lone worker policy, moving and handling.
4. Maintain and develop continual professional development in line with the NMC Revalidation process and contribute safe and effective clinical practice following Revalidation processes and The Code. The Registered Nurse is expected to improve and contribute to the continuous improvements to keep knowledge, skills and qualifications up to date in clinical and non-clinical areas.
5. Maintain professional communication with the Responsible Person and the senior Management Team within the Care Home. The Nurse must take every opportunity to discuss the quality of care and any clinical concerns that may arise and any issues relating to safeguarding in a timely manner.
6. Support residents to understand the Ageing Process and how it affects their health and lifestyle. Support the resident's to maximise their independence and create the environment which supports their physical and mental well-being and ensure all residents are treated with dignity and respect.
7. Adhere to All Wales Safeguarding policy and the reporting procedures. Contribute to any safeguarding investigations within the care home, following the Duty of Candour requirements.
8. Comply with Accident and Incident reporting policy to maintain the safety of the staff and residents in the care home, following the Duty of Candour Requirements.
9. Support all residents with their individual diagnosis who may require additional psychological or emotional support.
LEADERSHIP
1. To model Person Centred Care in your practice
2. Lead a Multi-disciplinary shift team, supervising staff in all aspects including orientation of new agency staff, allocating and co-ordinating team members to deliver care to the appropriate resident group and supervising the delivery of quality care delivered to residents providing support, supervision and mentoring as required within your scope of competence in compliance with the NMC the Code.
GENERAL
1. To be professional, courteous and transparent in your communication.
2. Develop close working relationships with other clinical staff in the team including the Senior Management Team
3. Any other reasonable task requested by your Clinical lead / Registered Manager / Administrator which is within your competence. You do have the right to refuse any task which is not within your level of competence
4. Support the Clinical Training programme in the care home through facilitating staff to attend informal and formal training opportunities. You are obliged to comply with all mandatory and statutory training requirements.
5. Assisting in the investigation of complaints, clinical or otherwise
6. To practice within the Policies and Procedures, The Statement of Purpose and Management strategies of Pentwyn house Care Home
WORKING CONDITIONS
1. This position is physically and mentally demanding, you will be expected to stand for extended periods of time and do repetitive tasks. Your employment rights to breaks are explained in your Statement of Particulars of Employment.
2. Concentrate in an intense and sometimes noisy environment.
3. Cope with potentially violent and threatening situations particularly in the EMI Communities.
