ROLE PURPOSE
The Frailty Coordinator plays a vital role in supporting older individuals living with frailty in the community. This role is designed to improve wellbeing and reduce isolation through the delivery of light personal care and the coordination of meaningful activities including arts, crafts, and gentle exercise.
You will build trusted relationships with service users and work creatively to support their independence, physical health, and emotional resilience.
KEY RESPONSIBILITIES
- Deliver light personal care in a respectful, person-centred manner.
- Plan and lead community-based sessions including:
o ARTS AND CRAFTS
o GENTLE EXERCISES
o REMINISCENCE AND SOCIAL ACTIVITIES
- Build meaningful relationships with service users to understand individual needs and aspirations.
- Promote emotional and mental wellbeing through social connection and structured support.
- Work collaboratively with families, carers, and external professionals to ensure joined-up care.
- Record and report progress and wellbeing outcomes.
- Contribute to service development through innovative ideas and continuous feedback.
PERSON SPECIFICATION
ESSENTIAL:
- Experience working in a care, support, or community setting (formal or informal).
- Relevant training or qualifications (e.g., Care Certificate, safeguarding, first aid).
- Compassionate and empathetic approach to supporting vulnerable adults.
- Confidence to deliver light personal care with sensitivity and respect.
- Creativity and confidence in planning and leading group activities.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Flexibility, reliability, and a proactive attitude.
DESIRABLE:
- Knowledge of frailty and age-related conditions.
- Previous experience supporting individuals experiencing isolation or loneliness.
