We have an exciting opportunity to join a committed and enthusiastic team working with people who have complex needs related to dementia and mental health illness. We welcome applicants at Band 5 for a developmental post into the band 6. The service is moving to follow me care principles within NHS England's Community Mental Healthcare Strategy. The post sits within the multidisciplinary community dementia team as a lead practitioner to deliver interventions for people with more complex or intensive needs, helping them to remain safe and well at home and maintain important relationships. The service offers extended hours and weekend interventions and operates 09:00 – 17:00, Monday to Friday. Applicants must be able to travel across sites.

Main duties of the job
The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to the care group, including education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work to promote safety and well‑being of service users and families/carers. They will assess, plan and implement care, provide specialist nursing advice and carry out specialist nursing procedures, and provide clinical supervision to staff and students.
Clinical duties include contributing to the management of a team caseload of clients with complex needs using CPA principles and evidence-based, client-centred approaches to assess, plan, implement and evaluate interventions delivered in the Dementia and Older Adults Mental Health Service; supporting/managing team caseloads; attending relevant MDT meetings; covering the duty rota; providing risk screening/assessment and risk management planning; contributing to evidence-based assessments and individual and group interventions; contributing to service maintenance and development; and supervising students on placement and junior staff as appropriate.
Qualifications
* Registered Nurse with current registration (mental health/LD/Adult) appropriate to the job role
* Completion of Mentorship Course/ENB equivalent
* Evidence of continuous professional development
Knowledge and Experience
* Significant post‑qualifying experience in the relevant care group
* Experience of working with people with complex health problems in the designated care group
* Experience of supervising staff
* Experience of providing a range of clinical interventions to people in the designated care group
* Experience of working consultatively with professionals and conducting clinical assessments including risk assessments
* Experience of partnership working across statutory and non-statutory teams and agencies
* Experience of working within the CPA process and understanding of clinical governance
* Previous experience of working in the community
