Overview
Adults Social Worker – Hospital Mental Health in Great Yarmouth, UK. This is a locum, full-time role with an ongoing contract. You will be working with a dedicated team, primarily focusing on The Julian Hospital, which has five specialised wards for older individuals with complex mental health disorders. You will also support Laurel Ward at Carlton Court in Lowestoft and be onsite at least two days per week.

What you will do
* Collaboratively work with inpatient and outpatient colleagues, community resources, and the care market to support safe patient discharges.
* Undertake Care Act assessments, Continuing Healthcare (CHC) assessments, and Section 117 planning, ensuring comprehensive patient advocacy.
* Lead complex reviews post-discharge in the community to ensure continued patient care and support.
* Engage with patients and multidisciplinary teams to identify and address individual needs.
* Attend essential face-to-face meetings, contributing to decision-making and care planning.
Requirements of the Social Worker
* Degree or equivalent qualification in social work.
* Current Social Work England registration.
* Significant frontline experience in the UK as a qualified Social Worker within Social Services.
* A good understanding of Mental Health legislation, though desirable, is not essential.
