What you will do:
* Conduct Eligibility Assessments: Undertake comprehensive assessments to determine children’s eligibility for support services by gathering and analyzing relevant information.
* Develop Care and Support Plans: Identify the impact of disabilities on children and their families, and create tailored care and support plans to ensure appropriate assistance.
* Collaboration and Communication: Work closely with families, healthcare professionals, and other stakeholders to ensure a holistic approach to care. Communicate effectively with children, parents, and carers to understand their needs and preferences.
* Service Development: Contribute to the development of services for children with disabilities by providing expert advice and insights. Participate in service planning and improvement initiatives to enhance care quality.
* Record Keeping and Reporting: Maintain accurate and up-to-date records of assessments and care plans. Prepare reports and provide updates for internal and external stakeholders.
* Professional Development: Stay updated with best practices, policies, and legislation related to children with disabilities. Engage in ongoing training opportunities to enhance your skills and knowledge.
Essential Requirements:
* Experience: Proven experience working with children with disabilities.
* Qualifications: Qualified Social Worker with a relevant degree (e.g., BA Social Work, DipSW, or equivalent).
* Registration: Registered with Social Care Wales (or willingness to register).
* Skills: Strong assessment and planning skills, excellent communication and interpersonal abilities, ability to work independently and as part of a team, and proficiency in IT systems for record-keeping and reporting.
Desirable Requirements:
* Knowledge: Familiarity with relevant legislation and policies affecting children with disabilities.
* Experience: Previous experience in a similar non-case holding role or in conducting assessments.
