Inflammatory Bowel Disease (IBD) Specialist Nurse

An exciting opportunity has arisen for a full-time nurse to join the Inflammatory Bowel Disease (IBD) nursing team within a Gastroenterology Specialist Nursing role.

Following training and supervision from the Senior IBD Specialist Nurse, you will work autonomously and as part of a multidisciplinary team (MDT) in both outpatient and inpatient settings to provide evidence-based care and support for newly diagnosed and existing IBD patients.

There will be an opportunity to complete an online IBD Nursing Education Programme along with other training workshops as they become available.

Crohn's disease and Ulcerative Colitis are relapsing and remitting lifelong chronic conditions where patients often require prompt support and escalation of treatment to avoid hospital admission and/or complications of their disease. The IBD Specialist Nurse plays a pivotal role in supporting patients with the physical, social and emotional impact of these conditions.

Main duties of the job

The role of the IBD nurse includes telephone reviews, managing an advice line, nurse-led clinics, attending and contributing to MDT meetings, medication monitoring, liaising with Primary/secondary and tertiary healthcare professionals and supporting patients, relatives and other HCPs in the area of IBD.

The majority of the working day involves telephone review and advice to patients. Therefore excellent communication and record keeping skills are essential for this position, along with significant experience as a registered Nurse within a gastroenterology related position, and with knowledge and a genuine interest in IBD.

Job responsibilities

Support and contribute to the delivery and development of the IBD nursing service.

Deputise in the absence of the Senior IBD specialist Nurse.

Act as a resource for education and clinical expertise related to IBD.

Provide quality, evidence-based healthcare and information for both newly diagnosed and existing IBD patients enabling the patient to make informed decisions regarding treatments and ongoing care.

Liaise effectively with all members of the multidisciplinary team to achieve a coordinated specialist nursing service providing excellent standards of care and access to specialist advice when patients are flaring.

Contribute to an advanced/expert specialist service that positively impacts on the experience of patients by providing high standards of clinical practice and care.

Manage safe and effective communication with the patients through phone calls and emails providing evidence-based advice and accurately recording outcomes.

Run IBD follow-up clinics that offer accurate, evidence-based advice on treatment, surgery, monitoring and lifestyle and to have the ability to provide the patients with well informed and up-to-date information about their disease.

Safely monitor patients on Thiopurines and Biologic medications ensuring that the results of blood tests are reviewed and acted upon according to the appropriate monitoring protocol. Where necessary to take appropriate action to ensure the patients safety, feeding back all actions taken to the Senior IBD Nurse and the appropriate consultant.

Apply for funding for high-cost drugs and keep the funding up to date following MDT decisions or the patient review.

Keep Health care delivery companies and the Infusion Suite informed when the patients are no longer using or are switching high-cost medications.

Ensure that there are accurate records and that GPs have been informed when a patient is changing the dose or type of medication.

Offer support to the wards for inpatients needing advice, information or to commence the patient on appropriate medications. Offer a timely review and ensure the patient has all necessary information and the contact details for the IBD Service.

Following up patients from endoscopy to ensure that patients are reviewed in a timely manner and those patients who are symptomatic are contacted as a priority and offered advice and treatment.

Offer education on IBD to other departments within the hospital to encourage the best possible care for IBD patients in accordance with the IBD Standards.

With the Senior IBD Specialist Nurse contribute to clinical governance within the Trust in IBD care and related areas via best practice development initiatives.

Contribute to the care of the patients by participating in discussion at the IBDMDT and the Biologics MDT running these meetings and acting on the outcomes in the absence of the Senior IBD Specialist Nurse.

Ensure that patients that report significantly deteriorating symptoms are reviewed in the appropriate setting in a timely manner, maintaining excellent communication with the relevant departments and clinicians. Ensuring that adequate information regarding the admission is passed on to the department allowing the patient to receive efficient and appropriate care.


Person Specification


Qualifications and Further Training

* First level Registered Nurse. Diploma or equivalent knowledge gained through significant experience of nursing as a Registered Nurse on a Gastroenterology Ward or other relevant area.
* First level degree in related subject or undertaking a Degree Pathway or equivalent training or qualifications.
* Experience of teaching and assessing.
* Evidence of knowledge in Inflammatory Bowel Disease.
* Teaching and assessing qualification.

Experience and knowledge

* Substantial post-registration experience in Gastroenterology related departments.
* Anatomy and physiology of the GI tract.
* Must be able to demonstrate that you have knowledge of your professional code of conduct and guidance issued by your professional body and that you adhere to these.
* Knowledge of inflammatory bowel disease, symptoms, treatments and IBD standards.
* Operating telephone advice lines.


Skills

* Able to provide a customer-focussed service.
* Good communication skills including ability to listen, to discuss and to inform clearly, ability to record in writing both clearly and accurately.
* Basic IT skills, including the use of Word, Excel and Outlook.
* Work independently, self-motivate, and manage time effectively in your role.
