We are seeking a Community Nursing Team Lead to work within the South Bedfordshire Locality.

You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills.

Detailed job description and main responsibilities

There is one part time 21 hours per week job.

Due to the nature of the post the applicant must be able to meet the travel requirements of the post.

Person specification

Education

Essential criteria

1. Registered General Nurse
2. Educated to degree level or equivalent
3. Post-registration qualification in clinical field eg: SPQDN (or working towards SPQDN)
4. Nurse Prescriber
5. Relevant teaching/mentoring qualification and evidence of teaching skills
6. Demonstrable evidence of significant post-registration nursing experience in a community setting.

Experience

Essential criteria

7. Evidence of and commitment to multi-professional working
8. Demonstrate working within a multi-agency environment
9. Have a comprehensive understanding of partnership working
10. Demonstrate application of clinical governance
11. Excellent understanding of community health and social care principles and practice
12. Understanding of National and Local issues that impact on the local population. Comprehensive understanding of the roles of others in health and social care.
13. Experience in managing multi-disciplinary staff.
14. Experience in managing issues related to conflict resolution avoidance.
15. Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
16. Knowledge of the principles of Health Promotion and prevention of illness
17. Experience of using electronic systems to manage activity and the delivery of KPI information.
18. Evidence of specialist holistic assessment and evaluation skills
19. Ability to prioritize and manage own time to meet the needs of the service
20. Ability to collect and interpret data in relation to patient activity and nursing care
21. Demonstrate experience in monitoring of staff and quality of service provided.

Knowledge and skills

Essential criteria

22. Demonstrate evidence-based practice and up-to-date clinical skills
23. Experience of clinical leadership
24. Extensive experience of complex assessment of health and social needs
25. Ability to oversee multiple caseloads and monitor quality
26. Demonstrate excellent communication skills using a variety techniques and strategies, i.e. email, documentation, active listening, verbal.
27. Ability to disseminate complex sensitive information to others
