Personal Adviser – Full Time, Permanent
The Post 21 Leaving Care team based at the Care Experienced Hub in central Bristol is looking for a Personal Adviser to lead statutory assessment, planning and support for care experienced young people from the age of sixteen through to adulthood.
Responsibilities
- Lead statutory assessment and preparation of pathway plans.
- Coordinate delivery of services through relationship‑based support.
- Provide direct work with care leavers, including writing reports and planning.
- Facilitate drop‑in and group sessions and workshops with multidisciplinary partners.
- Co‑create, write and support young people in developing pathway plans toward independence.
- Work 1:1 with care leavers and collaborate within multi‑professional networks.
- Travel to support care leavers across Bristol and surrounding areas as required; pool cars are available.
Qualifications & Skills
- Confidence in undertaking assessment of needs and writing reports and plans for young people.
- High level of direct work with young people; passionate, creative, organised and aspirational.
- Trauma informed and able to work independently while challenging constructively.
