Job Description:
An exciting opportunity has arisen for a Senior Social Worker and/or Approved Mental Health Professional to join the Older Peoples’ Community Mental Health Teams based at Drury Lane in Wakefield (West Team).
If you are not a qualified AMHP, training opportunities will be provided in the future.
Roles and Responsibilities
* Promote wellbeing and improve outcomes for individuals with mental health conditions as a Senior Social Worker.
* Collaborate with colleagues from Southwest Yorkshire Partnership NHS Foundation Trust.
* Work as part of an integrated team with other professionals and support staff.
* Help people live independently and empower them to make autonomous decisions regarding their care and support needs.
* Conduct assessments, including risk assessments, plan interventions, complete Care Act assessments, reviews, and develop person-centred, outcome-focused care plans.
* Commission services for people with mental health difficulties across West Wakefield.
* Work with individuals aged 65 and above experiencing mental health issues.
* Have experience working with mental health needs, possess excellent assessment and communication skills, and understand legislation such as the Care Act, Mental Capacity Act, and Mental Health Act.
* Complete post-qualifying awards or training, stay updated with professional expertise, and participate in team activities, including AMHP rota if qualified.
What will be expected from me?
* Manage a complex caseload and support team members and students.
* Apply a multidisciplinary approach to work in accordance with policies and procedures.
* Undertake statutory casework and interventions.
* Ensure effective enforcement actions when appropriate.
* Collaborate with health and social care professionals.
* Prepare reports on clients following policies and procedures.
* Hold a Degree in Social Work or equivalent recognized qualification.
* Maintain professional skills through ongoing training.
* Develop effective relationships with service users, carers, and groups.
* Possess excellent communication and engagement skills.
* Complete or be willing to attend AMHP training.
* Participate in professional development activities.
