<div><h3>Community Rehabilitation Nurse</h3><p>The closing date is 25 June 2026. This post is open to newly qualified professionals and students who are due to complete their qualification during this calendar year. Successful applicants may receive a conditional offer subject to completion of training and registration.</p><p>We have an exciting opportunity for two enthusiastic and highly motivated Mental Health Nurses to join the Community Mental Health Rehabilitation Team (CMHRT). The team is temporarily based at Engineers House on the St Austell Hospital site; however, as we deliver a countywide service, applications are welcomed from across Cornwall.</p><h3>Roles and Responsibilities</h3><ul><li>Work effectively both as a member of the multidisciplinary team and independently as a lone worker, in line with Trust policies.</li><li>Provide supervision and support to students and other learners within the team, ensuring practice is compliant with Trust and professional standards.</li><li>Maintain an up-to-date professional portfolio, including mandatory training, annual appraisal documentation, and supervision records.</li><li>Act as a mentor for student university placements, holding a recognised mentoring qualification or demonstrating willingness to work towards this and attend mentorship updates as required.</li><li>Deliver specialist and generic clinical interventions to enhance engagement with services, supporting service users and carers to access treatment that promotes wellbeing and recovery.</li><li>Undertake comprehensive assessments and develop holistic, person-centred care plans incorporating positive risk management and evidence-based interventions.</li><li>Ensure service users and carers are actively involved in assessment, formulation and care planning, demonstrating awareness of cultural and diversity factors and addressing barriers to accessing care.</li><li>Communicate effectively with professionals across disciplines and services, using a whole-team approach, and support service users to clearly and confidently express their needs.</li><li>Build and maintain therapeutic relationships that promote engagement and ongoing adherence to agreed care packages.</li></ul><h3>Qualifications and Experience</h3><ul><li>Current professional registration – NMC.</li><li>Working knowledge of crisis assessment and intervention.</li><li>Previous experience with community or acute services.</li><li>Previous experience within housing employment services.</li><li>Having the mentoring qualification or willingness to work towards it and attending mentorship updates as required.</li></ul><h3>Additional Requirements</h3><ul><li>Demonstrated ability to manage frequent travel across the county, including areas not accessible by public transport.</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and a disclosure to the Disclosure and Barring Service (formerly CRB) will be required to check for any previous criminal convictions.</p><p>Cornwall Partnership NHS Foundation Trust</p></div>
