As a member of the Community Cardiac Rehabilitation Service, the post holder will carry out administrative duties to support the Cardiac Rehabilitation Team and contribute to the delivery of cardiac rehabilitation across Calderdale.
The role involves providing highly skilled support to individuals referred to the Cardiac Rehabilitation Team, assisting with Cardiac Rehabilitation groups, contacting patients, and helping with office administration for the service.
In allocated time, the post holder will act as a point of contact for all communication for the team and work under the indirect supervision of registered practitioners. This includes taking responsibility for delegated workloads, following rehabilitation plans, making minor adjustments within agreed protocols, and liaising with the registered practitioner as necessary.
Responsibilities
* Assist with exercise assessments of cardiac patients
* Assist with and/or complete the exercise assessments at point of discharge
* Monitor progress and feed back to the registered practitioner
* Make minor adjustments to treatment programmes within pre‑determined limits
* Assist registered practitioners with complex treatments
* Assist with group exercise and advice sessions
* Provide highly skilled support to individuals referred to the Cardiac Rehabilitation Team
* Contact patients and support telephone enquiries in a sensitive manner, documenting and conveying messages to the team
* Arrange appointments for clinic consultations, home visits, and meetings (face‑to‑face or virtual)
* Process audit statistics and assist with inputting audit data onto the ongoing national audit for cardiac rehab
* Check email accounts, process referrals, and amend waiting lists for cardiac rehab
* Take notes and complete templates within patient notes at long‑COVID MDT meetings (if required)
* Print patient assessment packs including exercise sheets and information booklets
* Set up and clean down group venues before and after sessions
