We're looking for experienced Qualified Social Workers who are committed to putting people at the heart of everything they do.The RoleAs a Hospital Social Worker, you’ll work at the frontline of care planning and discharge, making a real difference in the lives of vulnerable adults. With your skills and expertise, you'll support timely and safe discharges, helping individuals regain independence and reduce their risk of harm.You’ll:
* Carry out person-centred assessments in a hospital setting.
* Use your professional judgment to create effective support and care plans.
* Work collaboratively with NHS colleagues, families, and other professionals.
* Ensure all interventions uphold dignity, empowerment, and best practice.
* Make informed decisions that lead to safe, sustainable outcomes.

Social Work England registered? Minimum of 2–4 years’ experience in Adult Social Care? Strong understanding of the Care Act, Mental Capacity Act & safeguarding frameworks? Excellent communication and relationship-building skills? Ability to manage a diverse caseload and prioritise effectively? Commitment to best practice, dignity, and empowermentDesirable Experience
* Hospital discharge planning or reablement
* Multi-disciplinary working across health and care
* Mentoring or supporting junior staff/students
