You will work within a team of 3 nurses and another experienced health care assistant.
You will be expected to have a basic knowledge of long term conditions, phlebotomy, life style knowledge and blood pressure procedures.

Main duties of the job
You will be closely working alongside a multidisciplinary team, to include GPs, clerical/administrative teams.
We have regular team meetings which you would be expected to attend, where appropriate. All opinions are valued and respected.
You will work alongside a ethically diverse team, to enable our patients to seek health care support and advice.
We adapt and change as required, with all the team included in quality improvement.

We are a practice of around 8200 patients. Around 60% of our patients require an interpreter.
The main spoken languages are Urdu, Arabic and Slovak.
Our team consists on 5 GP partners,3 salaried GPs, 3 nurses, 2 PAs and a health care assistant.
We are a training practice that supports growth and opportunities from within the team. Many of our team do dual roles to enable flexibility and enjoyment at work.

Qualifications
* GCSE grade A-C in Maths and English
* NVQ Level 2 in health and social care
* Willing to undertake home visits, clean driving licence required
* Experience in primary care
* Experience working within a diverse setting
