Responsibilities
* Manage a complex caseload of older adult clients in the community, ensuring their safety and wellbeing.
* Collaborate with clients, families, and carers to achieve identified goals and support recovery through a holistic approach.
* Accurately record clinical information into systems: risk assessments, care plans, crisis and contingency plans.
* Operate within our duty service, triaging referrals and signposting as needed.
* Work autonomously, planning and prioritising workload.
* Comply with legal and professional requirements, implementing and maintaining local and national policies.
* Demonstrate effective networking skills to collaborate with a range of services, external agencies and the voluntary sector.
* Participate in team supervision and support in flexible formats.
* Initiate, develop, and assist in trialing new ideas for service improvement, critically evaluating new concepts.
* Conduct memory assessments (clinical and home‑based), documenting outcomes; specialist training provided where needed.
Person Specification
Essential Qualifications
* RN (part 13) RMN, HCPC registered, OT or Physio or equivalent, or Social Worker.
Desirable Qualifications
* Clinical supervisor training.
Essential Experience
* Minimum of twelve months at Band 5 or equivalent mental health responsibility.
Desirable Experience
* Experience working with clients aged 65+ with mental health needs.
* Experience in Cognitive / Memory Assessment activity.
