In response to the high demand for services from our Hispanic/Latinx clients, the Community Behavioral Health Center (CBHC) in Lynn has developed "Equipo Renacer," a specialized team of Hispanic/Latinx providers. Our mission is to deliver high-quality, evidence-based mental health and substance use support that is culturally and linguistically attuned, respecting and valuing diverse cultural backgrounds and individual experiences. Responsibilities:
* Provide side-by-side interventions focusing on problem solving, skills training, modeling behavior, optimism, and encouragement
* Deliver interventions in the office, virtually, in the community, and in clients' residences based on client preference and clinical direction
* Enroll eligible individuals in care management, including BHCP and Health Home
* Collaborate with Care Transitions Team to triage hospital admissions data and develop follow-up strategies during inpatient stays
* Assist in ongoing assessment of symptoms and responses to interventions
* Participate in developing culturally competent, strength-based, realistic, and recovery-promoting treatment plans
* Ensure active involvement of individuals in their treatment planning, respecting their preferences
* Document progress in a measurable, objective, and clinical manner
* Coordinate timely face-to-face contact with enrollees following inpatient discharge, adhering to MassHealth requirements
Qualifications:
* Bachelor’s or Master’s degree in psychology, social work, or related field with at least three years of experience
* Strong commitment to recovery-oriented services
* Ability to establish supportive relationships with individuals with severe mental illnesses, children, and families, respecting their treatment preferences
* Effective communication skills with individuals in rehabilitation and treatment
Salary: $53,000 annually, including a 6% bilingual differential.
Hours: Monday-Friday, 9am-5pm, with remote/hybrid flexibility (3 days on-site, 2 days remote)
