An exciting opportunity has arisen for a motivated registered nurse to join the Anti-coagulation Service at SWFT as an Anti-coagulation Specialist Nurse. The role involves caring for patients at risk of or being treated for thrombosis or emboli.
Responsibilities include assessment, initiation, management, and evaluation of patients on anticoagulation therapy (including Low molecular weight heparin, direct oral anticoagulants – DOACs, warfarin, and other vitamin K antagonists). The service offers various clinics, including face-to-face and telephone consultations, with plans to expand inpatient support. The role involves clerical duties requiring good organizational skills and the ability to work methodically and meet deadlines.
The nurse will provide expert advice and interventions within the anticoagulation service, working within a multi-professional framework to maintain and improve standards of care. The primary role involves working with the Raid computerised dosing system, acting as a clinical expert alongside a Band 6 Nurse, and supervising support staff within the department.
