Clinical Support Worker - Out Patient Services

* SALARY: £12,147 (FTE £25,306)
* HOURS: 18 hours over 3 days
* DEPARTMENT: Out-Patient Services
* REPORTS TO: Registered Nurse - Out-Patient Services

Kilbryde Hospice provides specialist palliative care services to the people of South Lanarkshire. It is a specialist resource for those individuals receiving palliative care who have particularly complex needs. The Hospice strives to meet the physical, emotional, social and spiritual needs of patients and carers, offering support in coping with the practical implications of life-limiting illnesses. Staff are required to provide the highest standard of patient-centred care possible, while promoting choice, dignity, self-esteem, and an enhanced quality of life. The Hospice is a Company Limited by guarantee, and a registered charity, and is required by separate and differing legislation to meet finance, governance, and regulatory requirements.


JOB PURPOSE

As part of the multidisciplinary team, the post holder will assist in the provision of direct patient care in Day Services and where necessary across clinical services at Kilbryde Hospice. Under the direction of senior members of multi-disciplinary team (MDT) the post-holder will carry out duties as required to contribute to effective communication and provision of enablement care in the hospice. The post holder will always work with care, compassion, respect, dignity, and commitment.


KEY ACCOUNTABILITIES

• To assist senior MDT members with a range of duties as directed across hospice services

• As part of wider MDT be responsible for the assessment, care and treatment of patient/carer, and accurate completion of associated documentation.

• Maintain therapeutic relationships with patients, carers, relatives, volunteers, and colleagues.

• Ensure effective communication verbal, non-verbal, electronic, and written with staff, volunteers, patients and carers.

• Contribute to multi-disciplinary service delivery.

• Actively promote a rehabilitative model of care

• Contribute to and undertake ongoing education and research.


Patient / Carer Support

• Under direction of senior MDT members assist with all activities of daily living

• Under the direction of MDT, support carers via assessment

• Participate in patient and carer education sessions

• Undertake an enablement approach to care

• Ensure patients/carers are treated with dignity and respect at all times.

• Recognise any changes in patient/carer’s condition and feedback to relevant senior MDT members.

• Assist with moving and handling as required.

• Ensure safe and responsible use of equipment in relation to patient care.

• Assist with nutritional requirements.

• Facilitate and participate in self-management workshops for patients and carers.

• Encourage patient/carer to take an active role in their own health and wellbeing.


Communication

• Demonstrate effective communication skills at all times.

• Contribute to a multi-disciplinary team approach to communication.

• Involve patients and carers in care and treatment plan.

• Ensure written and electronic documentation is completed confidentially, accurately, and legibly in accordance with Kilbryde Hospice and Caldicott Guidelines

• Use sensitivity when dealing with bereaved relatives and families under guidance from senior MDT members.


GENERAL RESPONSIBILITIES OF ALL EMPLOYEES

• Have a good understanding of and always comply with relevant Kilbryde Hospice Policies and Procedures.

• Kilbryde Hospice is committed to encouraging volunteering throughout the organisation and as such the post holder will be expected to support and work effectively with volunteers.

• All staff are required to comply with the obligation of confidentiality relating to personal information that could identify individuals. GDPR (2018) safeguards the handling of information held in both electronic and manual filing systems and it is the duty of all staff employed by Kilbryde Hospice to uphold the principles of the legislation.

• All employees of Kilbryde Hospice must be aware of infection prevention and control policies and are expected to follow them at all times. Any breach of infection control policies which places patients, visitors or colleagues at risk may result in further action.

• The post holder is required to familiarise themselves with and comply with the Kilbryde Hospice Health & Safety policies.

• The post holder must always carry out duties and responsibilities with regard to the Kilbryde Hospice Code of Conduct.

• Kilbryde Hospice operates a strict non-smoking policy in the hospice or Hairmyres hospital grounds.

• The post-holder must at all times carry out their responsibilities in line with Kilbryde Hospice Dignity at Work, and Equality, Diversity & Inclusion policies.

Applicants should be aware that the role includes working with vulnerable groups and as such successful applicants will be subject to PVG check by Disclosure Scotland.


KNOWLEDGE, TECHNICAL SKILLS, QUALIFICATIONS, EXPERIENCE


Essential

• SVQ Level 2 minimum, Health & Social Care.

• Experience of working with patients requiring specialist palliative care

• Have an understanding of enablement within specialist palliative care.

• Possess strong listening and communication skills.

• Experience in working with Microsoft Office,

• Be able to work as part of MDT while having the confidence to work with patients, carers, and relatives on a one-to-one basis.

• Be able to treat all patients, carers/relatives and work colleagues with dignity and respect, irrespective of their gender, race, disability, sexual orientation, age or religion.

• Ability to carry out assigned patient care and tasks effectively within a busy environment.

• Ability to work a variety of shifts/internal rotation.


Desirable

• Experience of working in a hospice or enabling environment.

• Experience of undertaking workplace audits

• Experience of working with Excel and PowerPoint
