Client Coordinator

The role is based in our registered office, Monday to Friday, 9am-5pm. You will also be part of the on-call rota which involves taking the on-call phone home on some evenings and weekends. Being a driver is essential for the role.

Our care coordinators are allocated a specific area of responsibility and by working closely with each other provide the day-to-day support for our care team.
We are seeking a dedicated and organized individual to manage and coordinate individual care packages, ensuring that our clients receive high quality, person centred care tailored to their specific needs. In this role, you will work closely with clients, their families and other healthcare professionals. This is an excellent opportunity for someone who is passionate about delivering compassionate care and making a meaningful impact.
Duties of the Client Coordinator include, but are not limited to:
* Answering incoming calls in a positive, happy, professional, unrushed and pleasant manner
* Completing an effective care consultation to ensure the individuals care needs are well documented
* Writing personalised care plans for new clients, based on individualised needs for HCAs to follow including positive risk taking where appropriate.
* Introduce HCAs to new clients and ensure they understand the client's needs and wishes
* Completing all client quality assurance checks as per the QA schedule
* Completing all client service reviews and care plan revisions as per the QA schedule
* Follow up any actions required as identified in QA and service review
* Doing interim service reviews when required due to changes in needs
* Sending referrals to other allied agencies when the need is identified by any staff member
* Build positive collaborative relationships with our partner organisations such as OTs, DNs, GPs etc.
* Have confidence to effectively challenge a situation/decision to maintain a clients independence within their own home
* Ensure all our clients are happy and have a great experience of their care
* Participate in a share of the on-call rota out of hours
Skills and experience required for the role:
* You must have a minimum of 2 years experience in the health and social care sector for this position
* You must live within 20 minutes of Penrith
* You must have a valid UK driving license and access to your own vehicle
* You must be comfortable delivering hands on care if required
* You must have a good level of computer / IT proficiency
* Being caring and kind with a strong ability to connect with people and build relationships is important
