Occupational Therapist (Community Mental Health & Learning Disabilities)

Salary: Up to £37,000 + £2,400 car allowance

Location: Home based delivering in the community throughout the West Midlands

Working Hours: 9am to 5pm, on call every 5 weeks

Closing Date: 31st January 2026


Introduction to the role

We are looking for a creative and enthusiastic Occupational Therapist with a desire to make a difference in the lives of the people we support. You will work holistically, focusing on short, medium and long‑term goals across all aspects of daily life. Our team includes Occupational Therapists, RMNs, RNLDs, a Psychologically Informed Therapist and Operational Service Managers.

Your role will provide the opportunity to apply your current skills, develop as an autonomous practitioner and contribute to evidence‑based assessment and intervention for people with complex needs who are supported to live independently.

Under the Positive Behaviour Support Model, you will collaborate with all internal and external stakeholders to deliver robust, positive, evidence‑based support.


Key Responsibilities

* Completion of holistic Occupational Therapy Assessment using the MOHOST, Screening Tools and Activity Analysis
* Setting goals and planning interventions with the people we support and their team
* Delivering creative interventions using evidence‑based practices and approaches
* Evaluating the efficacy of interventions
* Caring for a caseload of approximately 15 people
* Providing assessment and intervention outcomes that contribute to MDT discussion, promoting collaborative working towards meaningful engagement and improved quality of life
* Evidencing therapeutic input by providing statistics relating to caseload
* Managing own time and priorities under the supervision of the Head of Rehabilitation
* Acting as a specialist clinical resource and demonstrating good leadership skills
* Taking a solution‑focused approach to mitigating risk of occupational deprivation
* Facilitating workshops and group discussions using appropriate delivery methods
* Working within CQC regulated activities and in line with MCA, Safeguarding and Code of Conduct policy and procedure


Key Accountabilities

* A commitment to undertake continuous training and professional development
* The ability to travel and undertake occasional overnight stays (car allowance and expenses paid)
* Being an ambassador for Gray Healthcare
* Providing clinical advice, guidance and support
* Working within a team that promotes positive risk taking
* Assessing, planning, implementing and evaluating clinical care plans and risk assessments
* Coaching and guiding support staff to provide robust and evidence‑based intervention with an emphasis on skill building
* Working collaboratively with key stakeholders, the individuals we support and their families
* Working with individuals and their families


Essential Knowledge, Skills & Experience

* Degree in Occupational Therapy
* Evidence of relevant experience relating to Mental Health, Learning Disability, Complex Needs, Autism and / or Acquired Brain Injury
* Minimum of 2 year post‑qualification experience
* Good assessment, planning, intervention and evaluation skills
* Evidence of CPD
* Experience working with a multi‑disciplinary team
* Excellent written and verbal communication skills including use of Microsoft Word, PowerPoint, Excel and Outlook
* Ability to demonstrate efficient and effective self‑management including use of time, organisation, planning and problem solving
* Ability to use own initiative but ask for assistance where appropriate
* Ability to use self‑reflection to improve clinical skills
* Commitment to continuous professional development
* Empathy and passion in working with clients with complex needs
* Access to a car and business insurance
* Ability to work flexibly to meet the needs of the service
* Ability to commit to overnight stays where required


Desirable Knowledge, Skills & Experience

* A working knowledge of the Positive Behaviour Support Model and how this enhances quality of life
* Experience working with the Model of Human Occupation
* The ability to work collaboratively with others, including the Learning and Development Team, Clinical Team, Operations Team and Quality Team
* An understanding of behaviours perceived as challenging and the ability to use this in supporting staff to be solution‑focused
* Experience working with electronic care planning programmes
* Good administration and organisational skills
* Awareness of Sensory Integration
* Understanding of CQC regulated activities
