What you will do:- Complete initial and ongoing assessments in accordance with legislative and local guidelines, ensuring adult voices are heard and respected.- Deliver person-centred, strengths-based practices that empower adults to lead fulfilling lives.- Develop and implement comprehensive care and support plans using resources that span community, informal, and statutory services.- Manage and review cases effectively, ensuring timely reassessments and maintaining high-quality records.- Lead efforts to safeguard adults from abuse and neglect, ensuring their safety and well-being.- Collaborate with partner agencies to achieve optimal outcomes for service users.- Actively engage in team processes, including supervision and case allocation, to foster a cohesive working environment.
