An exciting opportunity has arisen for a Band 3 Senior Support Worker to join Hillcroft, within the Trust's Learning Disabilities Directorate.
Hillcroft is a residential service for up to six individuals who have a learning disability, autism and additional health needs.
We provide intensive support and a wide range of activities for the people living at Hillcroft. We have excellent facilities. Hillcroft also boasts its own large secluded grounds. The service is very busy with fast pace working .
We are looking for an enthusiastic and passionate team members who are focussed on promoting people's rights, choices and independence and ensuring they live full and active life.
The post will offer excellent opportunities for development.
Ideally the post-holder will hold a full driving license.
Excellent communication skills and ability to effectively lead the shift by delegating fairly tasks allocated for the day and ensuring good team work.

Main duties of the job
* Support people using the service with their individual preferences to promote and engage in a meaning individualised activity programme which promotes as much independence as possible.
* To assist with all aspects of personal care (supporting continence needs, showering, bathing, etc) in accordance with individual’s support needs.
* Ensuring with creative, motivational planning that high standards of activities and excursions is planned and executed, and that new activities are introduced as appropriate.
* Reporting and recording all relevant information and reporting to senior staff any change in the person.
* Actively contribute to the review and development of plans, assessments and reports.
* Liaising, with the staff team, manager, day services, our local community team and external agencies in relation to activities.
* Administer and support individuals with their medication needs in line with the services requirements.
* Supervise support workers within the service, providing support and guidance on good, physical and motivational practice.
* To provide person centred support, listening to the people who live at Hillcroft. Also ensure along side their care plans, respect, privacy and dignity at all times.
* Help maintain and develop relationships between individuals, families, friends and significant others.
