Position Details
* Job title: Relief Support Practitioner | Care at Home Service
* Responsible to: Team Leaders and Service Manager
* Hours of work: Flexible, including mornings, evenings and weekends
* Salary: £12.60 per hour plus annual leave allowance
* Location: Field‑based working across all three localities of Aberdeen City (base office Bridge of Don)

Main Responsibilities
Lead and participate in the provision of a high standard of person‑led care and support for service users in their own homes, in line with individual care plans and risk assessments, organisational policies, national standards, codes of practice and legislation.
Communicate effectively with service users, families and multi‑disciplinary teams, record appropriate care notes and create a safe, positive and enabling environment.

Requirements
* Understand the physical, emotional, cultural and social needs of the service users.
* Provide wide range of domiciliary care including personal, continence care, medication management, household tasks and meal preparation.
* Knowledge of palliative and end‑of‑life care and working with vulnerable elderly people.
* Person‑centred, enabling approach; promote independence, dignity, privacy and rights.
* Support other Support Practitioners and use trauma‑informed practices.
* Provide input into care and support plans, use safe manual handling and equipment, and ensure safe handling of property.
* Maintain good communication with service users, families, multi‑disciplinary teams and facilitate effective working relationships.
* Ensure safe living environment respecting user choice and rights.
Essential Qualifications
* Minimum qualification of N/SVQ3 in Adult Health and Social Care, or equivalent professional experience and willingness to work towards SVQ3.
* Literacy and numeracy at level to maintain all appropriate paperwork and report writing.
* Ability to use smartphone app and computer applications such as Word.
* Proven track record in a similar role.

* Maintain records using PASS for all information related to Service User Case Management.
* Attend team meetings and use Office 365 and other IT platforms.
