Job overview

A three-month Band 6 secondment opportunity is available within the High Intensity User (HIU) Programme in Ealing, hosted by West London NHS Trust.

This role is ideal for high-performing Care Coordinators, Link Workers, or Social Prescribers looking to develop their skills and experience within an integrated, multi-disciplinary environment.

The post holder will work across Ealing Neighbourhood HIU MDTs, supporting coordinated care for service users with complex needs, strengthening referral pathways from non-primary care settings (Acute, LAS, Community Nursing Teams, RISE, MINT, etc…), and promoting holistic, person-centred support. The role also includes leading the coordination of a two-monthly Ealing-wide HIU Steering Group meeting.

Main duties of the job

§Working as an integral part of a multi-disciplinary team within the Ealing INTs

§Liaising with key stakeholders from the INTs to ensure a coordinated approach to service users care

§Coordination of the relevant HIU MDTs across Ealing INTs

§Capturing and recording key actions agreed for each service user discussed at the MDT

§Working with allocated case manager for each service user discussed at the MDT, coproducing a social prescription, which provides the individual with positive health and well-being outcomes through addressing non-medical/social issues in their life, such as addiction, housing, debt or isolation.

§Creating an innovative way of supporting services users on the integrated HIU caseload.

The role will be the fundamental link between the service user and the MDT ensuring and reporting over a time limited period on the progress and completion of agreed goals outlined in the service user care plan.

Essential criteria

1. Knowledge of the relevant field acquired through relevant degree, supplemented by specialist knowledge, acquired through training, extended courses and experience to post graduate diploma level equivalent.

Experience

2. Extensive experience and knowledge of working with vulnerable people.
3. Experience in managing proactive and reactive workloads.

4. Evidence of planning and delivering programmes, projects and services on time.

8. Proficient use of Microsoft Office suite, including Excel and PowerPoint.

10. Ability to engage and work with individuals from Diverse backgrounds.
