Terms and conditions apply Birkenhead Community Mental Health Service are seeking a number of enthusiastic community mental health nurses to join our busy team. Do you have a passion for supporting people within their own home to live as independently as possible? You will be joining a busy community mental health team who provides support to adults with a serious mental health condition such as Bi Polar Affective Disorder, Schizophrenia, Severe Depression to maintain their quality of life and remain well in the community. We are ideally looking for a practitioner with experience of both functional and organic mental ill health. However, we would consider a nurse with experience of one of these areas as training and support will be given to develop your skills and experience. This is a great time to be joining the team, as you will be part of developing our response to Community Mental Health Transformation. The post holder will work autonomously within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness. The post holder will maintain a caseload which will involve ongoing assessment: Formulation of care planning, risk assessments. Completing reviews and offering interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies as necessary. Excellent communication, report writing, and assessment skills are essential for this post. Applicants should have sound working knowledge of mental health disorders and the Mental Health Act. Supervision and training are mandatory with wider training / personal development readily available. Access to transport for work purposes is essential. Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire. We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton and Liverpool. We also provide specialist services for the North West as a whole.
