Position Overview
Coordinates and oversees all aspects of resident focused delivery of care. Provide direct supervision, leadership and education to other members of the health care team. Acts as a resource and advocate for residents, families, and staff. Promote the values, goals, and philosophy of the Home. Promotes a safe and secure environment for all residents.
Essential Duties
1. Development and modification of the care plans in response to residents’ change in status
2. Liaises with other healthcare personnel, agencies, and families
3. Maintains good relationships with Medical/Support/Home, staff, residents, families, and general public
4. Engages in many meaningful interactions with residents during your entire shift
5. Supervises all activities in his/her unit
6. Monitors performance and completes performance evaluation for the PSW’s
7. Directs, assigns, and evaluates the work of the PSW’s
8. May be in charge of Home as required
9. Utilizes leadership skills to identify problems and problem-solving on the unit
10. Monitors that care delivered is consistent with the resident Bill of Rights
11. Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported
12. Required to participate in the improvement of the indicators and achieve satisfactory results
13. Actively listens to residents' and family concerns, take appropriate actions to resolve the issue in accordance with policies of the Home
14. Conducts unit rounds at the beginning and end of the shift
15. Processes Physician orders and administers medications and treatments in compliance with the policies of the Pharmacy provider, the Home, and the Standard of the College of Nurses
16. Documents and reports all errors and omissions
17. Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
18. Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) and interdisciplinary team
19. Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team
20. Inspires others to be motivated, work with passion, and empower staff
21. Demonstrate understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home
22. Any and all other tasks as assigned
Qualifications
Current Certificate of Competence with the College of Nurses of Ontario as Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
Basic CPR
Experience in long term care setting an asset
Ability to work with and relate to interdisciplinary team members
Ability to work from the heart and make others smile
Well-developed written and oral communication skills
Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired
Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references

Preferred Bachelors or better in Science
