Main duties of the job
The post is designed to support line management to the Discharge team.
The post is required to support both patients and ward staff in the application of discharging patients with complex health and social care needs.
The Post holder will provide on-going education and development to staff across the Trust in relation to discharge planning and the development of an innovative service, to support the provision of excellent discharge practises within the ward environment.

Detailed job description and main responsibilities
Once your pre-employment checks have been completed you will be contacted by Recruitment to arrange your start date which will be in line with one of our Corporate Inductions.
Following your Corporate Induction you will be expected to attend the Trust’s 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. The programme is full-time (please email if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.
Please note that appointment to this post will be subject to a satisfactory Disclosure & Barring Service Check at Enhanced Level. (Enhanced level now includes regulated checks against vulnerable adults and children)

Qualifications
Essential criteria
1. Nursing degree
2. 1st Level Registered nurse NMC registration
Desirable criteria
3. Experience of working within the discharge field, knowledge of community services and social care.
4. Experience of working as a senior team member.
Knowledge/Experience
Essential criteria
5. A good understanding of the importance of discharge planning and the discharge process and a good knowledge base of welfare and benefits rights, overseas patients, homeless patients.
6. Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations
7. Supporting others (mentor / educator / supervisor ) experience

8. Evidence of innovation in previous roles

9. Excellent and effective communication skills, written and oral.
10. Ability to converse in a positive manner with all levels of staff, patients and relatives/carers.
