Overview
IMPACT is the East Midlands area Provider Collaborative formed to coordinate and deliver care for adult patients requiring low and medium secure care. It is led by Nottinghamshire Healthcare NHS FT as the Lead Provider, with IMPACT’s commissioning hub providing arms-length commissioning and clinical oversight. IMPACT sub-contracts other providers to support discharges, assure quality of services, and report regionally and nationally to NHSE. The post holder will support the IMPACT commissioning hub with case management coordination and quality oversight of care and treatment for patients within adult secure care across the East Midlands. The IMPACT Quality Manager will be an integral part of the IMPACT commissioning hub, supporting clinical, operational and quality oversight for the patient population within adult secure care. This role involves monitoring service improvement and supporting patients along their care pathway across the provider collaborative.
Responsibilities
* Support the IMPACT commissioning hub with coordination of case management activities and quality oversight for adult secure care across the East Midlands.
* Act as an integral part of the IMPACT team, supporting clinical, operational and quality oversight for the patient population within adult secure care.
* Monitor service improvement and support patients along their care pathway in collaboration with IMPACT and partner organisations.
* Engage with local clinicians and Experts by Experience to help design improved pathways of care.
* Prepare and contribute to reporting regionally and nationally to NHSE as required by the Lead Provider arrangements.
Qualifications / Requirements
* A full UK driving license and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
* Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment (DBS standard £26.40 or enhanced £54.40). The DBS application cost will be deducted over the first 2 months of employment.
* You are encouraged to enrol for the DBS Update Service (annual fee £16).
